Maternity Referrals For Booking With Community Midwives
In response to women and Community Midwives being unable to access the GP surgeries to complete a Maternity Booking Referral form we have set up an online system for women to inform the Community Midwives that they are pregnant. The email address will be checked regularly.
How does it work:
- Women send an email to the designated email address. Birthafterthoughts.cav@wales.nhs.uk
- They receive an automatic response with the referral form attached.
- They complete the form online and return it to the same email address.
- They will be contacted within a week by a Community Midwife who will then arrange a telephone booking appointment when they are 8-10 weeks pregnant.
